We are IN NETWORK with almost all major insurance plans. We also accept auto insurance and work comp insurance. Please use the following links to print and fill out our insurance coverage and patient health history forms.

New Patient Forms

We file all insurance paperwork on your behalf! Simply bring your insurance card with you and our staff will handle the rest. We currently accept and are in-network with the following insurance companies:

If your plan is not listed above, we may still take your insurance. Please call the office at 303.300.0424 for more information. Our self-pay rates are affordable and intended to make our care accessible to everyone. We also accept HSA / FSA / and HRA payments.

GROUP OR INDIVIDUAL INSURANCE

We will work directly with your insurance company to minimize any red tape. We’ll verify your coverage benefits when you come into our office. Please remember, however, that the benefits quoted to us by your insurance company are not a guarantee of payment. We will file your claim and await payment for 90 days. You are responsible deductibles, co-pays or co-insurance at the time of service. If you have any questions at all regarding you financial responsibilities at our office, please do not hesitate to ask. Our staff will be happy to assist you.

PATIENTS WITHOUT INSURANCE

We understand that some people don’t have insurance or have very high deductible plans. Therefore, we have affordable fees so those patients may receive complete care without undue financial difficulty. We do not do long-term treatment plans, nor do we require you to sign a contract. We accept cash, personal checks, Visa, MasterCard, and Discover cards.

Office visit (including first visit) $60.
6-visit package, $330 – ($55/visit – save $5/visit)
12-visit package: $600 – ($50 / visit – save $10/visit)
X-Ray self-pay fee: $40.

PERSONAL INJURY OR AUTOMOBILE ACCIDENTS

A car accident can be a significant disruption to your daily life. We are happy to help you recover from injuries sustained in an auto accident. Please bring your claim number, insurance adjustor’s phone number, and any other documentation relating to the accident and any other treatment you had. We will notify your insurance adjustor of your treatment, and will bill them directly. If you are attorney represented we will work with your attorney.

We accept Med Pay and all auto insurance policies for treatment. Although you are ultimately responsible for your bill, we will wait for settlement of your claim for up to six months after your care is completed. Once the claim is settled or if you suspend or terminate care, any fees for services are due immediately.

MEDICARE

We do not presently accept assignment from Medicare.

SECONDARY INSURANCE

Please inform us of any secondary insurance you may have. We will assist you if you need help in filing.
